In order to comply with U.S. Homeland Security requirements the Georgia General Assembly has changed the licensing process for all licenses issued by our state. Beginning July 1, 2013 all applications for commercial and private pesticide applicator licenses will have to comply with the lawful presence requirements shown below. Here is a step by step:
- Upon completion of the pesticide training CD and test, go to the following link http://www.agr.georgia.gov/verification-of-lawful-presence.aspx On the right side of the screen there is a “Documents” column. At the bottom of the column click the “Affidavit” link.
- Print the HB125 affidavit.
- The licensee or license applicant will complete the document and have it notarized at a public notary.
- Our Extension Office will: Send, fax or email the document to the Licensing Division or send it in with the license application. The GDA cannot process a license without the affidavit completed and notarized.
It is not the responsibility of the county Extension office to verify citizenship or the authenticity of identification documents. It is the applicant’s responsibility to provide a notarized affidavit. It is the notary’s responsibility to affirm that the applicant presented what appeared to be legitimate identification documents and that the applicant signed the affidavit. As always, the Extension agent will provide training and document the fact that the applicant received that training and correctly answered questions as described in Georgia’s pesticide applicator certification plan. Extension will gladly submit the applicant’s affidavit with the training documentation, but accepts no responsibility for any falsified documentation that an applicant might present.
For Private Pesticide Applicator licenses the County Offices are to copy the applicant's lawful presence documentation, along with their signed and notarized HB125 affidavits. Attached these items to the standard document verifying that the applicant completed the training and passed the examination. Send this package to the Georgia Department of Agriculture, Licensing Division.
