When: Thursday, April 6 from 1:00-4:00 pm
Who: Custodial parent or legal guardian

You MUST bring the following to register:
Copy of certified birth certificate
Copy of Georgia immunization certificate (form 3231)
Copy of Georgia Eye, Ear, Dental, Nutrition screening
(form 3300)
Copy of child's social security card
Driver's license or state issued ID of custodial parent or legal guardian

Trion City Schools district residents MUST provide a current power, gas, or water bill in the custodial parent/legal guardian's name. It is NOT necessary that the child be present for registration. A testing appointment for a later time will be scheduled on the day of registration.
